Clarity360 is being developed to bring those pieces together into a clearer, more useful picture so people can better understand what may be connected, what matters most, and what to focus on next.

More information does not always create more clarity.

People living with multiple, overlapping, rare, or not-yet-clearly-explained health concerns often accumulate a great deal of information over time.

But that information may live in different records, specialties, diagnoses, conversations, and parts of life.

It can be hard to answer seemingly simple questions:

What is actually going on?

Which parts of my history matter most?

What might be connected?

Are there patterns that cut across several concerns?

What should I prioritize? What can I act on myself?

Where do I need support? How do I communicate the whole picture clearly?

Clarity360 is designed for the space between having health information and being able to understand and use it as a whole.

Clarity360 uses a structured whole-person approach to organize and synthesize relevant health information.

The goal is not to generate more data. It is to make existing information more useful.

How it works

Relevant information may include: Health history; current symptoms and concerns; diagnoses; medications and therapeutic supports; labs and other testing; family history; major health and life events; nutrition, movement, sleep, stress, and other everyday influences; and the individual’s own experience, priorities, and questions.

Look across the whole picture

Rather than treating each issue as an isolated problem, Clarity360 looks for meaningful patterns, relationships, shared influences, and areas that may deserve closer attention.

STEP 3

Turn understanding into action

The result is organized into practical tools intended to help people identify priorities, clarify questions, and determine reasonable next steps.

What you receive

My Health Story

A clearer picture of what is happening.

A concise, organized synthesis of the health story, including important history, current concerns, meaningful patterns, and connections worth understanding more deeply. The Health Story is intended to help someone see their own experience as a whole rather than as a collection of disconnected problems.

My Action Plan

A clearer sense of where to focus next.

A prioritized set of opportunities and next steps based on the issues and patterns identified through the process. Actions may include things someone can explore independently, questions to bring to a clinician, areas where more information may help, professional support, and practical health and lifestyle opportunities.

The aim is to reduce the feeling of “there are a hundred things I could do” into a more focused place to begin.

Tools designed to help translate the Health Story into practical support.

For clinical support: a focused guide that highlights relevant history, patterns, priorities, and questions to support more efficient and useful clinical conversations. For non-clinical support: a separate guide designed to help individuals identify practical next steps, available resources, and types of support that may be useful to seek when coordinated care is not readily available.

The goal is not to assume that everyone has a care team. It is to help people use their Health Story more effectively within the support system they actually have access to, especially in rural, underserved, or resource-limited communities.

Health does not always fit neatly into one box.

Clarity360 is being designed for people who may be navigating:

You do not need to have a diagnosis for every concern. And you do not need to have an established multidisciplinary care team to benefit from organizing and understanding your health story.

It starts with the whole person, not a single condition.

Many useful health tools are organized around one diagnosis, symptom, specialty, or type of data. Clarity360 intentionally looks across those boundaries.

Clarity360 provides health education, information organization, and structured synthesis. It does not diagnose medical conditions, prescribe treatment, or replace evaluation and care from qualified healthcare professionals.

Some opportunities identified through the process may be appropriate for an individual to explore independently. Others may require discussion, evaluation, testing, or treatment with a clinician or other qualified professional.
